Privacy Statement – Stichting Ouder- en Kindteams

Ouder- en Kindteams Amsterdam consists of Stichting Ouder- en Kindteams (responsible for youth care) and GGD and SAG (responsible for youth health care). This privacy statement applies to the activities of Stichting Ouder- en Kindteams. For the privacy statements of GGD and SAG, please refer to their respective websites.

 

Stichting Ouder- en Kindteams operates independently and confidentially in the field of youth care (1). We handle your and your child’s personal data with great care, as explained in this privacy statement.

 

Questions or Concerns About Health, Growing Up, or Parenting

If you have questions or concerns about health, growing up, or parenting, you are always welcome at the Ouder- en Kindteam in your neighborhood or through the ouder- en kindadviseur (parent and child advisor) at your child’s school. You can reach out for tips, information, and advice.

 

Courses

One of the recommendations may be to participate in a training. When registering through our website, we request some personal data (2) in order to organize the training for you. This data is shared with the trainers. These trainers may be from Stichting Ouder- en Kindteams Amsterdam, or from training organizations we collaborate with. Once the data is no longer needed, it will be destroyed.

 

Tailored Support – Always in Consultation

If tailored support is needed, you will work with one or more of our staff members. This is usually a parent and child advisor, but may also involve an orthopedagogue or GZ psychologist. Together with the professional(s), you will determine what is needed, who can help, and what works best for your situation. Support will only start with your consent. Everything is done in consultation so you know what to expect. Without your consent, no information will be shared with others, unless an exceptional situation requires it.

 

Plan and File

Together with the parent and child advisor, you’ll assess the situation and create a plan. This plan will be documented in a (digital) file according to legal requirements. The file includes personal data necessary for providing care. It is securely stored and accessible only to professionals from the Ouder- en Kindteam who are involved in your child’s case.

  • If your child is under 12 years old, you as a parent may always access the file.
  • If something in the file is incorrect, you can request a correction.
  • If your child is between 12 and 16 years old, both you and your child can access the file and request changes.
  • From the age of 16, your child is responsible for accessing and managing the file.
     

Consultation with Others

If a staff member of the Ouder- en Kindteam wishes to consult with others to ensure proper care, they will request consent in advance.

  • If your child is under 12, you as a parent must give consent.
  • If your child is between 12 and 16, consent must be given jointly by parent and child.
  • From age 16, your child gives consent independently.
     

Sharing Information with Others

If a staff member wants to share information with other care partners, your consent will also be requested in advance. You will be informed:

who the information will be shared with (e.g., general practitioner, school, or specialist); what specific information will be shared; why this is considered necessary.
 

Exceptions

In some cases, we are required to act without prior consent. If we are concerned about a child’s safety, we act in accordance with the Domestic Violence and Child Abuse Reporting Code (Meldcode Huiselijk Geweld en Kindermishandeling). Parents are informed about this. When concerns or risks are present, a staff member will register this in the Multisignaal reference index. More information can be found at multisignaal.nl.

We may also receive information from another organization that must be included in the file. You will be informed if this occurs.

 

Referral

If your child requires specialized care, the staff member from the Ouder- en Kindteam will arrange an appropriate referral in consultation with other youth care providers. A formal decision will be issued, including the child’s name, citizen service number (BSN), and the type of care required. You will receive a copy of this decision, which entitles you to the specified care.

 

Data Security and Retention

We have implemented appropriate security measures to protect your personal data. After the legal retention period of 20 years, the file is destroyed. We have agreements in place with the IT companies that support our systems to ensure information security.

 

Your Rights

You have the right to access, correct, or delete the personal data we hold about you. You also have the right to withdraw any previously given consent or object to the processing of your data. To exercise these rights, please contact us using the contact information below.

 

Questions or Complaints About Privacy

Do you have a question about how we handle your personal data? Are you not satisfied with our approach, or do you have a suggestion for improvement? Please contact your local Ouder- en Kindteam.

We have appointed a Data Protection Officer (Functionaris Gegevensbescherming) to advise our organization and oversee compliance with privacy laws and regulations. You can contact our Data Protection Officer at privacy@oktamsterdam.nl.

If you are unable to reach a resolution with your local Ouder- en Kindteam or our Data Protection Officer, you can contact our complaints support service at klacht@oktamsterdam.nl.

If you are still not satisfied, you can submit a complaint to the Dutch Data Protection Authority (Autoriteit Persoonsgegevens).
